Transfer Your Care to North St Ives Medical
We look forward to providing you with seamless, ongoing care. Transferring your records from another practice helps us understand your medical history and deliver the highest standard of care from day one.
Note: If you’re transferring records for immediate family members aged 16 or younger, you can include everyone on a single form. Otherwise, each patient over 16 must complete their own form.
Complete the digital form
This lets your former practice know exactly what information they need and how to send it.Forward the PDF to your former practice
Once you submit the digital form, you’ll receive a PDF by email. Forward that PDF (or print and hand-deliver it) to the practice that currently holds your records.Sit back and relax
Our team will confirm receipt of your records by email once they arrive at our practice.
Transfer to Another Practice
We understand that moving to a new GP can feel overwhelming. At North St Ives Medical, our priority is to ensure your transition is as smooth and stress-free as possible. Follow these simple steps to send your records to your new provider:
Obtain a transfer request form from your new practice
Ask your new GP for their transfer request form. This ensures we know exactly what details they need and how to send your records.
Complete the attached form
• Upload a clear scan or photo of the signed request.
• Pay the applicable transfer administration fee.
Sit back and relax
Once we receive your completed form and payment, we’ll process your transfer within 10 days. Your new practice will then arrange to receive your records directly from us.

There’s no cost to you — it’s fully funded by Medicare.

When booking online, the system will guide you to schedule two appointments on the same day — one with our practice nurse, and one with your preferred GP.

My Medicare

You can register for MyMedicare in a way that suits you:
Option 1 – Quick & Easy (Electronic Form)
When you book an appointment with us, we’ll send you a link to complete an electronic registration form.
If you fill it out, we’ll finalise the enrolment on your behalf — no further action needed.
Option 2 – Paper Form
Prefer to register in person? Just ask our reception team for a paper form next time you visit.
Sign the form and we’ll handle the rest.
