This is a Medicare-funded, in-depth check-up conducted by your GP and practice nurse, focusing on your physical, mental, and social wellbeing.

If you’re aged 75 or older, this assessment is for you — especially if:

• You’re supporting a loved one and want to ensure they have the right care

This is a comprehensive, personalised check-up designed around your goals and lifestyle. It may include:

There’s no cost to you — it’s fully funded by Medicare.
