My Medicare Registration
MyMedicare is a new way to strengthen the connection between you and your regular healthcare team — and it’s completely voluntary and free to register.
When you register with North St Ives Medical Practice through MyMedicare, it formally notifies us that you’ve chosen our team as your regular provider of care. This helps us offer you better continuity, follow-up, and access to important services.
1. Why Register?
Patients who register in MyMedicare will have access to:
Greater continuity of care with your regular practice, which can lead to better health outcomes
Access to longer Medicare-funded telehealth consultations with your GP
Future enhancements to chronic disease management and care coordination
If you choose not to register, you’ll still receive the same high standard of care from our team — MyMedicare simply gives you access to additional benefits.
1. How to Register
You can register for MyMedicare in a way that suits you:
Option 1 – Quick & Easy (Electronic Form)
When you book an appointment with us, we’ll send you a link to complete an electronic registration form. If you fill it out, we’ll finalise the enrolment on your behalf — no further action needed.
Option 2 – Paper Form
Prefer to register in person? Just ask our reception team for a paper form next time you visit. Sign the form and we’ll handle the rest.
Option 3 – Self-Register Online
You can also start the process yourself via:

Make sure North St Ives Medical Practice is listed as your chosen clinic. Once submitted, our staff will confirm and complete the registration.

You’ll receive a personalised health summary to guide your care and support your independence.
4. Want to Provide Extra Info?
As part of your registration, you’ll have the option to share some extra details about your health. This helps us, and the broader health system, better plan and improve the care you receive. You can still register even if you choose not to share this information.
